Release and Waiver of Liability

I acknowledge that I have voluntarily agreed to allow my child or children to attend one or more Camp or Program at the Souris Valley Museum (Souris Valley Museum Inc.). By signing this form, I agree to allow my child or children to attend the Camp or Program.

I also agree to release the Souris Valley Museum (Souris Valley Museum Inc.), and its employees from any liability if the child suffers any injury in the course of the program's activities. I forever release the Souris Valley Museum (Souris Valley Museum Inc.), its employees, affiliates and representatives from any and all actions, claims, or demands that I, my assignees, heirs, guardians, next of kin, spouse and legal representation now have, or may in the future, for injury, death or property damage, related to my child's participation in these activities, the negligence or other acts, whether directly related to these activities or not, and however caused, by any Releasee, or the condition of the premises where these activities occur.

I also agree that I, my assignees, heirs, guardians, next of kin, spouse and legal representation will now make a claim against or bring legal action against any Releasee in connection with any of the matter covered by foregoing release.

I understand the Souris Valley Museum (Souris Valley Museum Inc.) reserves the right to refuse participation in any Camp or Program to any child not made subject to the terms and conditions of this waiver.
